Extension appears in Extension Manager BUT not in List of Available extensions in Photoshop

I installed a Photoshop extension with the 2015 extension cc manager. The extension shows in the list of extensions in the manager. However, when I go to Photoshop.Window->Extensions -- it doesnt show in the list of Photoshop extensions, so I can't use the extension. Any guidance on how to fix or work around this?
